All Volunteers will be required to sign this waiver to participate in the event:

VOLUNTEER SIGN-IN & / MEDIA RELEASE / WAIVER OF LIABILITY

I understand that I am participating in Senior Wish Day as a volunteer.  I understand that, as a volunteer, I may be involved in physical activities that have a potential risk of injury.  I assume that risk.  I agree that I will only perform volunteer activities that I am comfortable and qualified to do.  I also agree that I will not hold Costa Bella Builders or Team Samurai or The Volunteer Center of Santa Cruz County and all of their former and current officers, directors, shareholders and employees, any of their community service partners, volunteers or the Senior(s) being served by me responsible or liable for any damage or injury to me or my property as a result of my participation in Senior Wish Day.  I also grant full permission for organizers to use photographs or footage of me in legitimate accounts and promotions of this event.

All Seniors Recipients will be required to sign this waiver to participate in the event:

HOMEOWNER HOLD HARMLESS AGREEMENT

I (we) am requesting that the Senior Wish Day program assist me (us) by performing minor repairs in my (our) home or on my property. I (we) understand this service will be provided entirely by volunteers.

By my (our) signature(s) below, I (we) agree not to hold the Senior Wish Day program, Costa Bella Builders, Team Samurai, The Volunteer Center of Santa Cruz County, or event volunteers, liable for any claims arising from personal injury, property damage, loss or claim of whatever nature resulting from any inadequate or improper repairs, damages resulting therefrom, or accidents which might occur in connection with services rendered in the Senior Wish Day program, or for any damage directly or indirectly arising out of compliance or non-compliance with laws, government regulations, orders or requirements, or for any cause beyond their control.
